This consent explains what receiving care through telehealth involves. You will be asked to accept it, and the Medical Group's own consent forms, before a Clinician reviews your information. Please read it carefully and ask questions before you agree.

What telehealth is
Telehealth means a licensed Clinician of the Medical Group (ZMD of CA, P.C.) evaluates and treats you using secure electronic tools: an online questionnaire, photos, messages and, where required by your state, a video or phone visit. Clinicians may review your information asynchronously (not in real time). You will not have an in-person physical exam.
Possible benefits
- Convenient access to care without travel or waiting rooms.
- Ongoing follow-up and dose adjustments by message.
- Faster communication with your care team.
Risks and limitations
- A Clinician cannot examine you in person and relies on the information you provide; incomplete or inaccurate information can lead to wrong decisions.
- Technical problems may delay or interrupt care.
- Despite strong security, electronic information could be accessed without authorization.
- A Clinician may decide that telehealth is not appropriate for you and recommend in-person care.
Alternatives
You may choose in-person care with your own doctor, other weight-management programs, nutrition and activity programs, FDA-approved brand-name medicines through your doctor or insurance, or no treatment.
Consent to prescription treatment
If a Clinician prescribes a GLP-1 medication (such as compounded tirzepatide or semaglutide with B12 or glycine), you confirm that you have read the Important Safety Information, including the boxed warning about thyroid C-cell tumors, contraindications and side effects, and that you will follow the instructions you receive.
You understand that compounded medications are prepared by a state-licensed pharmacy for you individually, are not FDA-approved, and are not the same as, or generic versions of, brand-name products. Dissolving tablets and compounded blends were not studied in the clinical trials of brand-name medicines.
Some uses may be off-label, meaning the FDA has not approved that medicine, dose, route or combination for that purpose. Off-label prescribing is legal and common when a Clinician judges it appropriate.
Your responsibilities
- Give complete, truthful information about your health, medications, allergies and pregnancy status, and update it if anything changes.
- Use medication only as prescribed and only for yourself; store it as instructed.
- Report side effects and new symptoms to your care team promptly, and keep your primary care provider informed.
- Seek in-person or emergency care when needed.
Records, privacy and communications
Your information becomes part of your medical record and is protected as described in our HIPAA Notice and Privacy Policy. You agree to receive care-related communications by portal message, email and text message.
Pharmacy of your choice
You may ask for your prescription to be sent to a pharmacy of your choice where state law allows. Pricing and availability may differ.
Emergencies
Telehealth is not for emergencies. If you have a medical emergency, call 911. For a mental-health crisis, call or text 988.
Voluntary participation
Your participation is voluntary. You may withdraw this consent at any time by contacting care@rovymi.com or your care team, without affecting your right to future care or your rights under the law.
Acknowledgement
By accepting this consent you confirm that you have read and understood it, that you had the chance to ask questions, and that you agree to receive care through telehealth.
